29 CFR 1910.261(c)(9)(i): A flagman did not direct the movement of locomotives being moved across railroad tracks or roads, and at any points where the vision of the operator is restricted. The flagman was not in sight of the operator when the crane or locomotive is in motion: (a) Dock Area, Warehouse 10: On or about August 13, 2020, the employer did not ensure that a flagman was in sight of the engineer while directing him into the building. This condition exposed employees to struck-by hazards.
