29 CFR 1926.1204(i): The employer did not develop and implement procedures for summoning rescue and emergency services, for rescuing entrants from permit spaces, for providing necessary emergency services to rescued employees, and/or for preventing unauthorized personnel from attempting a rescue: On or about July 9, 2020, at or near the intersection of Mitchell St. and W. Barkley St. in Sour Lake, TX, where employees were exposed to toxic sewer gas and engulfment hazards while performing work in a sewer manhole without implementing procedures for summoning rescue and emergency services.
