29 CFR 1910.147(c)(7)(i)(A): Authorized employee(s) did not receive training in the recognition of applicable hazardous energy sources, the type and magnitude of the energy available in the workplace, and the methods and means necessary for energy isolation: A) Un-Scrambler and Stacker Area - On or about July 7, 2020 and/or times prior to, employees were not trained on how to perform lockout or tag out on equipment such as, but not limited to, the stacker or un-scrambler, thereby exposing employees to moving chain conveyors and in-running nip points.
