29 CFR 1910.146(f)(7): The entry permit that documented compliance and authorized entry to a permit space did not identify the hazards of the permit space to be entered: Employees were exposed to chemicals hazards. Employees vacuuming lime pebble and granular quicklime from a lime pit were exposed to dermal and inhalation hazards. The confined space entry permit for the L. G. Everist lime pit, located at 8440 Gibbs Rd. in Kansas City, KS 6611 did not identify lime exposure as a potential hazard.
