29 CFR 1910.1025(e)(4)(i): When ventilation was used to control exposure to lead, measurements which demonstrate the effectiveness of the system in controlling exposure were not made at least every three months: a. On or about January 22, 2020, at Line 39 Process Room, the employer did not conduct adequate ventilation measurements for the dross drum vacuum system. There was decreased effectiveness of the dross drum vacuum system in controlling employee exposure to Lead when the Lead pot was drossed, due to the following: 1. A fan belt was missing, 2. Multiple holes in flexible duct, 3. Fallen ductwork. b. On or about February 11, 2020, at Line 39 Process Room, the employer did not conduct adequate ventilation measurements for the dross drum vacuum system to discover an open end of a round duct near the Lead pot hood and this decreased the effectiveness of the dross drum vacuum system in controlling employee exposure to Lead when an employee drossed the Lead pot. Operator employees who fully drossed the Lead pot were exposed to airborne lead in excess of the Lead PEL.
