Location: 409 East 110th Street, New York, NY 10029 On or about: December 21, 2019 (a) The employer did not ensure employees acquired the knowledge and skills required for the safe application, usage, and removal of the energy controls. Employees were exposed to uncontrolled energy sources while performing maintenance on park lifts.
