29 CFR 1926.503(a)(2)(iii): The employer did not assure that each employee has been trained, as necessary, by a competent person in the correct use and operation of personal fall arrest equipment. a) Ruscilli Construction Company Inc. located at 230 E. Long Street, Columbus, OH 43215: On and preceding December 10, 2019, the employer did not ensure that employees were trained in the correct use personal fall arrest equipment at the job site. A vertical lifeline, rope grab was used as a personal fall arrest system (PFAS) by the assistant superintendent near 90 degree leading edges while installing guardrails and inspecting leading edge work at heights beyond 6 feet. The lifeline was not designed for use as a PFAS near leading edges. The following condition exposed the employee to falls exceeding 6 feet.
