29 CFR 1910.178(a)(4): Modifications and additions which affect capacity and safe operation shall not be performed by the customer or user without manufacturers prior written approval. Capacity, operation, and maintenance instruction plates, tags, or decals shall be changed accordingly: a. Facility - On or about November 19, 2019, the employer did not have prior written approval from the forklift manufacturer regarding modifications and additions related to the yellow fork extensions which was utilized on the Clark (model TM15S S/N TM127- 0269-4901) fork truck.
