29 CFR 1910.119(j)(4)(ii): Inspections and testing procedures performed on process equipment did not follow recognized and generally accepted good engineering practices (RAGAGEP): Avantor Performance Materials/Phillipsburg, NJ/Building 101-Methanol Distillation Process Area: a) Reboiler R-03 (2000 gallon still) On or about October 2, 2019, the employer failed to establish and implement inspection and testing procedures that follow the employers chosen RAGAGEP, such as but not limited to, API 510 (2016), Section 5.5.5.1 when the employer did not conduct external thickness inspection and testing procedures for the Reboiler, exposing employees to fire and explosion hazards in the event of a Methanol release. b) Receivers T10, T11 and T12 (2000 Gallon still) On or about October 2, 2019, the employer failed to establish and implement inspection and testing procedures that follow the employers chosen RAGAGEP, such as but not limited to, API 510 (2016), Section 5.5.5.1 when the employer did not conduct external thickness inspection and testing procedures for the methanol receivers, exposing employees to fire and explosion hazards in the event of a Methanol release.
