Text For Citation: 01 Item/Group: 005 Hazard:
29 CFR 1910.333(c)(3)(i)(A)(1): When an unqualified person was working in an elevated position near overhead lines with voltages to ground rated at 50 kV or below, the location was not such that the person and the longest conductive object he or she may contact cannot come closer to any unguarded or energized overhead line within 10 ft. (305 cm): The employer is failing to protect four contractor employees from electrical shock and electrocution hazards. This was most recently documented on or about September 6, 2019 when four contractor employees were working inside grain silo bin #258 located at 15662 258th Road, Cummings, Kansas. The contractor employees were exposed to electrical shock and electrocution hazards while preparing to exit the bin through the new construction opening on the south side of the grain bin silo. The 3 phase 480 volt power lines, on the outside of the bin, approximately six feet below the construction opening were not de-energized. A worker's wire rope lanyard made contact with a bracket containing 3 phase 480 volt power lines resulting in workers being shocked and one worker being electrocuted.
