29 CFR 1926.651(k)(1): Daily inspections of excavations, the adjacent areas, and protective systems were not made by a competent person for evidence of a situation that could have resulted in possible cave-ins, indications of failure of protective systems, hazardous atmospheres, or other hazardous conditions: (a) On or about 7/23/2019 - 4901 Twin Pine Circle, Hoover, AL, employees were exposed to cave-in and struck-by hazards when working in a trench and an inspection had not been made by a competent person. The trench measured approximately 5 to 7 feet deep, 70 feet long, and 3.5 to 5 feet wide with spoil piles up to 5 feet high located on either side and at the edge of the trench.
