29 CFR 1910.147(d)(4)(i): Lockout or tagout devices were not affixed to each energy isolating device by authorized employees: Workplace, NJ Route 440 and 22nd Street, Bayonne, NJ The employer did not ensure that each authorized employee performing flange slip removals on the 50% Sodium Hydroxide tank line above rail line 7 and 8 outbound loading pipe, and truck rack #2 outbound loading pipe placed their own individual locks into the lockbox used for the group lockout procedure. Violation occurred on or about 8-28-19
