29 CFR 1910.334(a)(2)(ii): If there is a defect or evidence of damage that might expose an employee to injury, the defective or damaged item shall be removed from service, and no employee may use it until repairs and tests necessary to render the equipment safe have been made: At 3311 Chouteau Avenue, St. Louis, Missouri, specifically inside of the Maintenance Shop, the outer insulation belonging to an orange, flexible cord supplying 120 volt electrical power to the Milwaukee Portable Hand Grinder, s/n H408D1829 09046, was damaged. This violation was noted on 08/14/19.
