29 CFR 1926.602(c)(1)(viii)(A): The industrial truck was not equipped with a safety platform/cage firmly secured to the lifting carriage and/or forks when used for lifting personnel: a) Intersection of Parkway Drive/Merritts Avenue NE: On or about June 6, 2019, and at times prior, the employer exposed employees to fall hazards of approximately 15 feet, in that a JLG forklift truck model 10054 (serial number 0610087797) was used to elevate employees on a job made work platform that was not firmly secured to the lifting carriage and/or forks while installing CAT-5 camera wires.
