29 CFR 1910.177(f): The employer did not establish a safe operating procedure for servicing multi-piece rim wheels which contained, at a minimum, the elements listed in 29 CFR 1910.177(f)(1) through (11) and did not ensure that employees follow the established safe operating procedures: The employer is falling to protect employees from struck-by hazards associated with foam filling a tire. This was last documented on or about 05/28/2019 at the Shamrock Tire & Auto Service, Inc. dba Tire Dealers Wholesale facility located at 3003 S. Broadway St. Wichita, Kansas 67216. The employer did not provide adequate operating procedures for foam filling mult-piece rim wheels to include proper use of restraining devices or alternative methods such as chains and straps and to ensure that employees followed safe operating procedures for servicing a multi-piece rim wheel.
