29 CFR 1910.212(a)(3)(ii): Point(s) of operation of machinery were not guarded to prevent employee(s) from having any part of their body in the danger zone(s) during operating cycle(s): The employer did not ensure the point of operation on the Waldemar Initial Pinch Bending Roll Machine had effective guarding: 1. On or about May 14, 2019, the roller machine was in operation and the equipments point of operation was not effectively protected against unintended or inadvertent contact by non-operators. 2. On or about May 16, 2019, the operator was exposed to caught in and amputation hazards on the roller machine because the point of operation did not prevent employees from having any part of their body in the danger zone during normal operations.
