1910.119(d)(3)(ii): The employer did not document that equipment complies with recognized and generally accepted good engineering practices. On or about 04/02/19, at the KMCO facility located in Crosby Texas, the employer did not document that equipment complied with recognized and generally accepted good engineering practice such as America Petroleum Institute (API) 521- 2007 Edition (Pressure-relieving and De-pressuring Systems) when it failed to ensure that pressure relief valve stacks that discharge to the atmosphere at least 15 meters (50 feet) horizontally from any structures for the following but not limited to: a) PSV-165 (or RV-17340) stack which protects isobutylene pressure vessel (T-2503) as depicted on New IB storage P&ID (Drawn 10/26/16) b) PSV-164 (or RV-17339) stack which protects isobutylene pressure vessel (T-2503) as depicted on New IB storage P&ID (Drawn 10/26/16) c) SRV- 4376 (or RV-21658) stack which protects propylene oxide pressure vessel (T-2010) as depicted PO Storage P&ID PG.1 (Drawn 11/23/16) d) SRV- 4375 stack which protects propylene oxide pressure vessel (T-2010) as depicted PO Storage P&ID PG.1 (Drawn 11/23/16) This condition exposes workers to the hazards of a catastrophic equipment failure including being struck by a high energy release, equipment parts and fire/explosion.
