29 CFR 1910.212(a)(2): Guards shall be affixed to the machine where possible and secured elsewhere if for any reason attachment to the machine is not possible. The guard shall be such that it does not offer an accident hazard in itself. a. On or about 2/21/19 the employer did not ensure that guards were effectively secured to the Viking packaging machine, which exposed employees to amputation hazards. The machine came with guards that had an interlock, but the interlock was bypassed by the contacts being taped together.
