29 CFR 1926.652(a)(1): Each employee in an excavation was not protected from cave-ins by an adequate protective system designed in accordance with 29 CFR 1926.652(b) or 1926.652(c): a) Sewer Tap Excavation/Tail Ditch - On or about December 7, 2018, employees were allowed to perform work in an excavation without adequate cave-in protection in Type B soil to connect a residential sewer line to the municipal water system, thus exposing employees to caught-in and engulfment hazards in an excavation that was up to about 9 feet deep.
