29 CFR 1915.71(j)(5): Toe boards of at least 1 x 4 inch lumber were not provided when they were necessary to prevent tools and materials from falling on men below. (a) Port of Toledo - F/V Pegasus - On or about November 14th, 2018 and at times prior thereto, JAG Industrial and Marine Services exposed its employees to contact with tools and material falling off of the topside of F/V Pegasus without toe boards in place to protect them.
