29 CFR 1910.178(p)(1): Powered industrial truck(s) found to be in need of repair, defective, or in any way unsafe had not been taken out of service until restored to safe operating condition(s). On October 30th 2018 in the B&H Freight Line Inc. warehouse located at 1700 Anaconda Road, Harrisonville Missouri 64701, employees were exposed to serious struck by injuries from the Toyota powered industrial vehicle 7FGCU25. The following items were found to be missing, or defective and in need of repairs: 1. Missing non-slip emergency brake pedal cover. 2. Inoperative Horn.
