29 CFR 1926.1053(b)(1): When portable ladders are used for access to an upper landing surface, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface to which the ladder is used to gain access: a) 203 Walker St., Lenox, MA: On or about October 17, 2018, an employee was observed using a portable extension ladder to access his roof top work location that did not extend the required 3 feet past the upper landing surface.
