29 CFR 1910.147(c)(7)(i)(A): Authorized employee(s) did not receive training in the recognition of applicable hazardous energy sources, the type and magnitude of the energy available in the workplace, and the methods and means necessary for energy isolation: a. On or about September 22, 2018, in the facility, the employer did not provide effective training to each employee to include all of the elements in (a) through (c), thereby exposing employees to unexpected re-energization of equipment. Press operators were required to use interlocks for isolation devices when entering hazardous locations such as the gated areas of the press lines and the basement under the press 7. Employees were thereby exposed to the unexpected start-up of the machines in the press lines.
