29 CFR 1910.119(j)(2): The employer did not establish and implement written procedures to maintain the on-going integrity of process equipment: a) Loading Rack, 436 Doremus Avenue, Newark, NJ Employees monitor and operate a covered process which includes 272,000 lbs. of pentane, a flammable liquefied petroleum gas. Written procedures were not developed to ensure on-going integrity of the covered process, after the pentane injection points through the Loading Rack portion of the covered process. Equipment in the Loading Rack includes, but is not limited to, piping, thermal pressure relief valves, emergency shut down (ESD) systems, foam extinguishing system, loading arms, dry break couplers, dispensing hoses, bottom loading couplers, Scully overfill protection and grounding systems, blending meters and control valves.
