29 CFR 1910.303(b)(7)(iv): There shall be no damaged parts that may adversely affect safe operation or mechanical strength of the equipment, such as parts that are broken, bent, cut, or deteriorated by corrosion, chemical action, or overheating. a. On or about September 21, 2018, located on the mezzanine at southwest wall, there was a Sander Automation electrical 480 VAC electrical cabinet that had a broken indoor panel disconnect requiring employees to open the door to access the panel while live, thereby exposing the employees to arc flash/blast and electrical shock hazards.
