29 CFR 1910.178(q)(6): Powered industrial trucks were altered so that the relative positions of the various parts were different from what they were when originally received from the manufacturer, by the addition of extra parts not provided by the manufacturer, or by the elimination of any parts without the manufacturer's approval: a) Facility - On or about 08/29/18 and at times prior thereto employees operated the Toyota cherry picker and climbed on top of a makeshift wood platform which was mounted loose on top of the forks. The platform measured 48 in. x 96 in., and extended 56 in. beyond the length of the forks. Or in the alternative: 29 CFR 1910.178(a)(4): Modifications and additions which affect capacity and safe operation of powered industrial truck were performed by the employer without the manufacturer's prior written approval: a) Facility - On or about 08/29/18 and at times prior thereto employees operated the Toyota cherry picker and climbed on top of a makeshift wood platform which was mounted loose on top of the forks. The platform measured 48 in. x 96 in., and extended 56 in. beyond the length of the forks. In accordance with 29 CFR 1903.19(d), abatement certification is required for this violation (using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET).
