29 CFR 1910.424(c)(2): Diver was not line tended from the surface or accompanied by another diver in the water in continuous visual contact during SCUBA diving operation: Pacific Ocean off the coast of the Silver Strand, 32.39.3962N/117.09.6871 (LAT/LONG): On August 22, 2018, employees were performing SCUBA diving operations to mark a sunken road line. A diver descended 29 feet to the ocean floor without being line tended from the surface. The diver suffered fatal injuries after he lost consciousness underwater and could not promptly be removed from the water.
