Text For Citation: 01 Item/Group: 005 Hazard:
29 CFR 1926.501(b)(4)(ii): Each employee on a walking/working surface shall be protected from tripping in or stepping into or through holes (including skylights) by covers. (a) On or about the 10 day period of July 17 - 30, 2018, located on the west side of a pipe and precast concrete plant, employees gaining access to elevated work areas for the purpose of demolition were exposed to trip and fall hazards while using fixed exterior stairs that contained an uncovered floor hole measuring 5 feet by 9 inches at the 2nd story stair landing. (b) On July 31, 2018 and at times prior thereto, located on the west side of a pipe and precast concrete plant, employees were exposed to trip and fall hazards while working at ground level near the base of a partially demolished building that contained an uncovered floor hole measuring 7 feet by 3 feet by 2 feet deep.
