29 CFR 1910.212(a)(3)(ii): Point(s) of operation of machinery were not guarded to prevent employee(s) from having any part of their body in the danger zone(s) during operating cycle(s): Fiber Baler: On, or about, July 9, 2018, adequate safeguarding was not provided where the hydraulic inserter pierced holes through the bale in order to insert wire. Wires broke during the automatic baling process several times per shift which required employees to manually thread the wire through to secure the bales.
