29 CFR 1910.146(k)(3): Whenever an authorized entrant entered a permit-required space, retrieval systems or methods were not used to facilitate non-entry rescue: a) On or about June 20, 2018, in Melt shop, employees were required to enter Vacuum Degasser, a permit-required confined space, without a retrieval system or other method to facilitate non-entry rescue. b) On or about June 22, 2018, Heat Treat Department, employees were required to enter the large heat treat tank, a permit- required confined space without a retrieval system or other method to facilitate non-entry rescue.
