29 CFR 1910.178(l)(4)(ii)(B): Refresher training in relevant topics was not provided to the operator when the operator had been involved in an accident or near-miss incident: On or about May 11, 2018, employees were exposed to struck by hazards when employees involved in accidents received disciplinary action but no refresher training in topics relevant to the accident. An employee who struck a column on 3/28/2018 had received disciplinary action but no retraining. In accordance with 29 CFR 1903.19(c), abatement certification is required for this violation (using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET).
