29 CFR 1910.147(c)(4)(i): Procedures were not developed, documented and utilized for the control of potentially hazardous energy when employees were engaged in activities covered by this section: a) On or about 4/18/2018, In the Bar Mill: Employees were performing various tasks including, but not limited to saw blade changes, grinding wheel changes, and cleaning operations while not utilizing lock out procedures that provided full protection against unintended re energizing of equipment. Employees were not maintaining control of lock out device keys. Abatement certification is required for this item.
