29 CFR 1918.24(e)(1): Ladders or steps furnished to obtain access to or from a stowed deck load or other cargo were not secured or held against shifting or slippage while in use. a) On or about April 11, 2018, at the aft barge, the portable ladder providing access to the stowed deck load and other cargo was not secured to prevent shifting or slippage when employees used it to access the area. Employees were exposed to falls of greater than 4 feet into the water below or onto the cargo deck.
