29 CFR 1910.28(b)(11)(ii): Each flight of stairs having at least 3 treads and at least 4 risers was not equipped with stair rail systems and handrails in accordance with Table D-2, Stairway Handrail Requirements: a) On or about 04/05/18 at the Lackawanna location; employees were exposed to a fall hazard when a 21 inch wide stair system provided access to the control shack, had four risers and three treads, was open on two sides and a lacked a stair rail system on each side. b) On or about 04/05/18 at the Lackawanna location; employees were exposed to a fall hazard when a 36 inch wide stair system provided access to the generator, had five risers and four treads, was open on two sides and lacked a stable stair rail protection on the south side and a stair rail on the north side that was not attached. ABATEMENT CERTIFICATION REQUIRED
