29 CFR 1910.213(a)(9): All belts, pulleys, gears, shafts, and moving parts were not guarded in accordance with the specific requirements of 1910.219: a) On or about 4/5/2018, Machine #4026, 1 head notcher did not have any guarding to protect operators from the saw blades or drive belts and pulleys. b) On or about 4/5/2018, Machine #4037, Printer did not have guarding to protect operators from amputation hazards created by open gears and roller nips. c) On or about 4/5/2018, Machine #4038, Large printer did not have guarding to cover 5" gap where gears are located or guards in place on backside of machine. d) On or about 4/5/2018, Conveyor #3033 did not have a back guard on the drive unit chain and sprocket exposing the operator to a potential amputation injury along with the ingoing nip point for the conveyor belt and pulley. e) On or about 4/5/2018, West Plains saws #3013 and #3022 did not have guarding to protect operator from reaching into saw blade and other moving parts which could cause an amputation. f) On or about 4/6/2018, Hotz Manufacturing gang saw paddles create an amputation point as they pass by shaft. g) On or about 4/6/2018, Two edge trim saw #8026 lacked guarding to prevent amputation injuries from contact with blades or unguarded belts and pulleys. h) On or about 4/6/2018. Pendu trim saw was not guarded to prevent blade contact on the front and back wall side which could lead to amputation injuries.
