29 CFR 1910.119(m)(1): The employer did not investigate each incident which resulted in, or could reasonably have resulted in a catastrophic release of highly hazardous chemical in the workplace: On or about 02/15/2018 at a time there after, in Area 320 of the CITGO Refinery in Lake Charles, the employer fail to investigate each incidents at the Unicracker witnessed by the employer management team. Instances: a. Non-functional instrument air gages on the Light Naphtha valve (LV052) was not investigated and track in the employers incident tracking system (IMPACT). b. Broken Eaton, Clouse-Hines pump switch on the Fractionator Reboiler Pump (JP8) was not investigated and tract in the employers incident tracking system (IMPACT). c. Open ended pipe on the seal pump system for the debutanizer reboiler pump (JP6A) was not investigated and tract in the employers incident tracking system (IMPACT). d. Conduit with exposed wires going to the light fixtures by the light product pump (JP10A) was not investigated and tract in the employers incident tracking system (IMPACT) e. Open ended pipe found above the Depentanizer reboiler (C108) associated with instrument number 320LSL083 was not investigated and tract in the employers incident tracking system (IMPACT)
