29 CFR 1910.178(l)(1)(ii): Prior to permitting an employee to operate a powered industrial truck (except for training purposes), the employer did not ensure that each operator had successfully completed the training required by 29 CFR 1910.178(l): (a) Throughout the establishment, employees operated a Toyota forklift truck without having first completed training consisting of formal instruction, practical training, and an evaluation of the operator's performance in the workplace. Employees were exposed to struck-by and/or crushed by hazards.
