29 CFR 1915.93(b)(2): The employer did not ensure that each employee uses a safety belt, securely and tightly fastened, at all times while operating or riding in a motor vehicle: (a) On the dry dock which was holding M/v Matanuska at Vigor Marine, LLC shipyard at 5555 N Channel Ave, Portland, Oregon: On or about 2/7/2018 employees were exposed to struck by and rollover hazards, in that a driver was operating an industrial truck #F30 without using a safety belt. (b) Outside of the lunchroom at Vigor Marine, LLC shipyard at 5555 N Channel Ave, Portland, Oregon: On or about 2/7/2018 employees were exposed to struck by and rollover hazards, in that a driver was operating an industrial truck #10576849 without using a safety belt.
