29 CFR 1910.212(a)(5): Fan blade guard(s) were not provided where the periphery of the blades was less than seven feet from the floor or working level: (a) Options Medical Center dba Options, 1700 S. 120th St., Lafayette, CO 80026: On or before January 25, 2018, the employer did not ensure that fans lower than seven feet from the floor were adequately guarded. Multiple fans in the Veg, and Flower areas had their front guards removed. This condition exposed employees to cuts and amputation hazards. Abatement Note: Abatement certification is required for this item (see enclosed "Certification of Corrective Action Worksheet").
