29 CFR 1910.334(a)(2)(i): Portable cord and plug connected electric equipment and flexible cord sets(extension cords) were not visually inspected before use on any shift for external defects (such as loose parts, deformed and missing pins, or damage to outer jacket or insulation) and for evidence of possible internal damage( such as pinched or crushed outer jacket): On or about December 28, 2017, at a workplace located at 2990 Tem Tex Blvd, Terrell, TX, employees working in the walls department where extension cords were missing a ground pin. (a) Employees were using a 12-feet orange extension cord that was missing a ground pin. The extension cord was used to power a Dewalt header DW260. (b) Employees were using a 12-feet yellow extension cord that was missing a ground pin. The extension cord was used to power a Dewalt, model no. 4011 hand grinder.
