29 CFR 1910.28(b)(1)(i): Each employee on a walking/working surface having an unprotected side or edge which was four feet (1.2m) or more above a lower level was not protected from falling by the use of guardrail systems, safety net systems, or personal fall arrest systems: Incoming Docking Area - On or about November 7, 2017, and at times prior to: a) at the loading dock adjacent to the emergency exit door, employees operating forklifts, packing pallet trucks, and moving boxes were exposed to a fall of approximately four feet and four inches where a single chain was installed across the dock door opening. The chain did not provide adequate protection to prevent employees from falling from the dock to the concrete below. b) at the loading dock where the dumpster was placed, employees discarding trash, packing pallet trucks, and moving boxes were exposed to a fall of approximately four feet and four inches where no guardrails were installed across the dock door opening. c) at the Sahara Coloring Machine: Employees performing maintenance on the Sahara Coloring Machine were exposed to falls in that a mid-rail was not installed on the platform above the machine. The distance between the existing top rail and the platform was approximately 36 inches.
