29 CFR 1910.119(j)(4)(ii): Inspection and testing procedures did not follow recognized and generally accepted good engineering practices (RAGAGEP): On or about 11/29/2017, at Petromax Refinery facility in Houston Texas, the employer did not establish corrosion rates in accordance with recognized and generally accepted good engineering practices such as America Petroleum Institute (API) Standard 510 - 2014 (Pressure Vessel Code: In-service Inspection, Rating, Repair, and Alteration), Section 7.1.2 for vessels including, but not limited to the following: a) D100: De-salter (P&ID 2366-P-D0008 Rev.6) b) T101: Crude Column (P&ID 2366-P-D0012/13 Rev.5) c) D105: LPG (Liquid Petroleum Gas) Accumulator (P&ID 2366-P-D0028 Rev.7) d) D108: LPG Storage tank (P&ID 2366-P-D0040 Rev.3)
