29 CFR 1910.147(d)(6): Prior to starting work on machines or equipment that have been locked out or tagged out, the employer did not ensure that the authorized employee verified that isolation and deenergization of the machine or equipment had been accomplished. On or about November 22, 2017, at the A&AT, LLC plant in La Porte, Texas, the employer failed to ensure employees followed energy control procedures to verify the Step 2 "C" Reactor isolation valve remained locked out/ tagged out and the isolation was validated prior to taking the blind off the piping connected to the reactor.
