29 CFR 1926.1053(b)(1): Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed: a) Construction site: On or about October 6, 2017, and at times prior, at 1679 Scenic Highway Snellville, GA 30078, the employer exposed employees to fall hazards of approximately 12' and 9'' in that employees did not extend the ladder three feet from the surface while framing a commercial structure.
