29 CFR 1926.1053(b)(8): Ladders placed in any location where they can be displaced by workplace activities or traffic, such as in passageways, doorways, or driveways, shall be secured to prevent accidental displacement, or a barricade shall be used to keep the activities or traffic away from the ladder. On or about September 25, 2017 located at 11332 Tamarco Dr Blue Ash Oh along a front overhead doorway entrance employees were gaining access to the metal decked roof area of the structure by utilizing a fiberglass extension ladder which was not secured from displacement or protected by barricade exposing employees to fall potentials in excess of 21 feet.
