29 CFR 1910.178(l)(6): The employer did not certify that each operator had been trained and evaluated as required by paragraph (l) of 29 CFR 1910.178. a) At 2307 US Hwy 31S, Bay Minette, AL: On or about September 19, 2017, the employer exposed employees to struck-by hazards in that employees were allowed to operate forklifts without the employer first ensuring the employees were competent to operate the equipment. The employer failed to certify in writing that employees known to have been previously trained, were trained on site specific hazards and forklifts at a new location.
