29 CFR 1910.212(a)(3)(ii): Point of operation guards were not designed and constructed as to prevent the operator from having any part of their body in the danger zone during the operating cycle: a) On or about 9/20/2017, Paxall cartoner for line 7A did not have interlocks functioning for guard doors to prevent operator from reaching into the danger zone during the operating cycle. b) On or about 9/20/2017, Bartelt machine for line 7A did not have guard doors secured by special tools as designed for closure by the manufacturer. c) On or about 9/20/2017, Line #25 had 3 guard doors missing (2 on west side of machine and 1 on east side of capper). Filler guard door interlocks were not working either. d) On or about 12/13/2017, Line 3B was being operated without interlocks on the guard doors to prevent operator from reaching into the danger zone during the operating cycle. Amputation occurred when employee opened guard door to remove pouch that was stuck on a clip without shutting off machine first.
