29 CFR 1910.178(l)(1)(i): The employer did not ensure that each powered industrial truck operator is competent to operate a powered industrial truck safely, as demonstrated by the successful completion of the training and evaluation specified in this paragraph (l): On or about August 09, 2017, employees were allowed to operate powered industrial trucks (e.g. the company's LP forklift) at the site without having received formal instruction, including operator evaluation to determine the retention of the information.
