29 CFR 1910.147(f)(3)(ii)(D): Each authorized employee did not affix a personal lockout or tagout device to the group lockout device before working on the machine or equipment: On or about 7/17/17 throughout the facility at locations including but not limited to the Monet saw, where the employer failed to enforce application of individual locks for each employee involved in activities including but not limited to maintenance tasks. Two Employees were observed exposed to potential caught-in and amputation hazards while changing a saw blade while utilizing only one lock to control the start-up of the saw.
