29 CFR 1910.146(k)(2)(iv): The employer did not ensure that affected employees practice making permit space rescues at least once every 12 months, by means of simulated rescue operations in which they remove dummies, manikins, or actual persons from the actual permit spaces or from representative permit spaces: a) In the North Shop, on or about July 20, 2017 - The employer failed to ensure that the emergency rescue team practiced making permit space rescues at least once every 12 months. Three employees lost consciousness inside the high pressure tank car TILX 303142 which contained an oxygen depleted atmosphere and needed to be rescued.
